Healthcare Network Expansion Strategy Across Muharraq and Riffa

Client

A regional healthcare operator engaged us to assess the expansion of its outpatient and diagnostic network in Bahrain. The client was considering locations across Muharraq, Riffa, Isa Town, and the wider Manama market.

Issues

The original expansion plan was based largely on population coverage and property availability. Management lacked a detailed understanding of patient catchments, competitor density, specialty gaps, utilization potential, and the degree of overlap that could develop between new facilities.

Opening several locations simultaneously would also create significant upfront capital exposure before demand had been fully validated.

Solution

We developed a healthcare network expansion strategy combining patient demand analysis, competitor mapping, catchment assessment, service-gap analysis, location screening, and financial scenario modelling.

Approach

We mapped existing healthcare providers and potential patient demand across the shortlisted locations, comparing accessibility, competitive intensity, service availability, catchment size, and commercial potential.

Alternative network configurations were then evaluated, including larger multi-specialty centers and smaller outpatient facilities supported by diagnostics, preventive care, and referral capabilities.

Recommendations

We recommended a phased two-location rollout rather than opening the full network simultaneously.

Muharraq was prioritized for the first new facility, followed by Riffa once predefined patient-volume and utilization thresholds had been achieved. The service mix was redesigned around selected outpatient specialties, diagnostics, and preventive care rather than duplicating hospital-level capabilities.

Engagement ROI

The revised rollout reduced modeled initial capital expenditure by 29% while increasing projected first-year facility utilization by 11 percentage points.

The recommended service mix brought modeled break-even forward by approximately seven months, while the phased rollout allowed the client to validate demand before committing additional capital.
